Pre-K 3 Registration For Students in Boucher or Faulk School Zones

The LPSS Early Childhood Programs would like to announce registration for Pre-K 3 on Wednesday, April 11, 2012. Students must meet the following requirements to register for the Pre-K 3 program.

• Must be 3 years old by September 30, 2012
• Must live in one of the following school zones:

• Alice Boucher Elementary
• J. W. Faulk Elementary

Registration will take place at the Vermilion Conference Center located at 326 Gauthier Road in Lafayette. (off of Pinhook Rd.)
Times: 8:30 a.m. to 2:30 p.m.

Please bring the following items with you when you register:

• Birth Certificate
• Social Security Card
• Up-to-Date Shot Records
• Parents' Proof of Income (pay stubs, food stamp printout, SSI, Disability)
• Parents' Proof of Address (light, gas or water bill/receipt)
