Changes to Medication Administration Policy
With the opening of the 2007-2008 school year, the Lafayette Parish School System has adopted revisions to the Medication Administration Policy. The revisions include the following: A new medication form and medication supply shall be provided each school year. It is the parent’s responsibility to make an appointment with the school nurse for an assessment to be performed on the student prior to medications being administered by school personnel. There will be scheduled assessment dates set up prior to the opening of the school year and times may be scheduled with the nurse for those medications that are ordered after the opening of school. No medication will be administered by school personnel until an assessment is completed by the school nurse. This also must be done each school year. If your child will receive medication during school hours, please make every effort to have all paperwork completed and contact the school nurse to schedule an appointment before school begins. You can reach the school nurse office at 234-1570. Paperwork may be obtained at lpssonline.com or in the school office. The nurses will be available for appointments the week prior to the start of school, August 6-10, 2007. When your appointment is made you will be given the time and location. For medication that is ordered after the start of school, please contact the school nurse office and an appointment will be scheduled. We appreciate your cooperation, as we continue to provide health related services to our students.
