All Professional School Counselors in the Lafayette Parish School System have at least a Master's Degree in Counseling. - Immunization (Shots) Record
- Proof of Residence (utility bill IN YOUR NAME, such as electricity, water, gas or sewage). If you have recently moved, a deposit/connection receipt is acceptable. If the utility account is not in your name, call us at 896-6127 for more information before coming to register your child.
- Social Security Card (or a copy).
- Custody Papers (if necessary).
- Last year's report card (if beginning of school year) or current grades.
- If coming from another school after the beginning of the school year, a withdrawal form from that school.
