Documents Needed for Registration

Please be sure that you have ALL the required documents before coming to register.

Needed documents

• Verification of address (must be current gas/electic/water bill with name/address printed on bill)
• Last report card (incoming 9th graders) or last report card and transcript (incoming 10th, 11th, or 12th graders) if entering during school year. Official withdrawal papers from the previous school are necessary.
• Immunization records
• Social security card
• Birth certificate
• Legal custody papers (if parents are separated or divorced, or if the guardian is a person other than the parents). Legal custody papers signed by a judge must be brought to the Office of Child Welfare and Attendance at the Lafayette Parish School Board Central Office.)
• LEAP test scores (incoming 9th graders). Incoming 9th graders who have not taken the LEAP test by the end of the 8th grade must contact Student Services at the Lafayette Parish School Board Central Office, 337-521-7081.
