Workers' Compensation Benefits

The Lafayette Parish School System operates a self-funded workers' compensation (WC) plan administered by FARA.  FARA handles the processing and monitoring of all WC claims.

For supervisors and their designees who must report a WC claim, please review the procedures for reporting a work-related injury or illness.    You can find the form for reporting a WC claim by clicking on the Insurance Forms link at the left of this page and clicking on the WC-1007 form.  A sample completed WC-1007 is provided to assist you as well as detailed instructions for completing the form.  If an injured worker needs to see a physician, you may provide him or her an Authorization to Treat form.
