Human Resources - Forms and Applications Welcome! On the following pages, we have gathered the most often requested forms and applications. The Lafayette Parish School System is accepting applications for the following positions year-round: Teacher Assistant, School Food Service Technicians, Custodians, Substitute Teachers, etc. Members of the public who are interested in applying for one of these positions, please find the appropriate Employment Application and directions on how to submit the application here. Current LPSS employees, please select Evaluation Forms to find all the Evaluation Forms currently used in our school system. Under Miscellaneous Forms, we have gathered the most commonly used forms our colleagues request: Change of Address Form, Direct Deposit Form, Federal W-4 Tax Form, State L-4 Tax Form, Resignation/Retirement Form and the Transfer Application Form.
