We have gathered the most commonly used forms that you, our colleagues request:

Tuition Exemption 2007-08 Teachers currently employed with the Lafayette Parish School Board wanting the following certifications are eligible to apply for 8(g) Local Teacher Quality Block Grant funds:  Currently on Out of Field Authority to Teach (OFAT); Seeking Highly Qualified Status in a core area being taught; Seeking Add on endorsements in SPED, Middle School ELA, Math, Science, Social Studies or Secondary Math, Science, Foreign Language.  In order to adequately determine your eligibility, you must complete the application in its entirety and submit by the deadline indicated.  Please use the complete name of the University in which you will enroll.  The application deadlines are as follows:

Spring 2008                 November 30, 2007                4:00 p.m.

Summer 2008               April 17, 2008                         4:00 p.m.

Fall 2008                      July 9, 2008                             4:00 p.m.
