4 months DTaP, Polio, Hib, & PCV-7
6 months DTaP, Polio, Hib, HBV, & PCV-7
12 months DTaP, MMR, Hib, Varicella, & PCV-7
4 years DTaP, Polio, & MMR

HBV - Hepatitis B Vaccine
DTaP - Diphtheria Tetanus Acellular Pertussis Vaccine
IPV - Polio Vaccine
HIB - Haemophilus Influenza Type B Vaccine
MMR - Measles, Mumps and Rubella Vaccine
PCV-7 - Pneumococcal Vaccine
Varicella - Chickenpox Vaccine
