School Requirements for Student Registration

  • A birth certificate
  • Proof of residence: a current utility bill (electricity/gas/water) in parent/guardian's name
  • A social security card (if available)
  • Student's withdrawal form from previous school, as well as last report card
  • Immunization records
  • All documentation for additional services needed: special education (IEP), 504 accommodations (IAP), medical needs, legal issues, etc.,

 

Non Domicilliary Verification Form -a form that must be completed by both parents if a student wishes to attend school in the zone of their non-domicilliary parent.

Notarized Statement Form -a form for families who live at an address where the utilities are not in there name.  In order to prove residence you must (1) complete this form and have it notarized (2) have a utility bill in the name of the individual who lives at that address (3) provide either a state or federal document that arrives addressed to you at that address OR a disconnect notice from your previous address.
