2017 Personal Health Assessment

Don’t forget to complete and log all wellness activities by 12/15/2017.  Employees who take a Personal Health Assessment, get an onsite wellness exam, and complete three additional approved activities by the deadline will be eligible to receive a $250 credit toward the 2018 annual deductible.

To verify that you have completed all requirements, please log onto the wellness portal at www.bluewellnessla.com.  After entering your username and password, click “Incentives” tab then “Incentives Summary” to view completed activities.
