Lafayette High Internship Participation Form

The Lafayette High School Academy of Health Careers is a program of instruction that ensures students selecting a career in health care have opportunities to meet their career and educational goals and are prepared to meet the demands of a rapidly changing professional world. The following are elements of the program: Partnerships with Health care professionals and agencies, Mentors/Guest Speakers to motivate and encourage students, School-to-Career emphasis allowing students to connect the relevance of school and the real world of professional health care, Cohort Scheduling with a team of Academy teachers in math, science, English, social studies, and their Health Academy elective course, and an Integrated focused curriculum where multiple course work in collaboration to make connections with academic materials and health care issues.

Students must enter the academy their freshman year or sophomore year. Students entering their sophomore year are required to take two academy courses. Applications are available online www.lafayettechoice.com/apply. Applications include an essay, parent permission, recommendations from a science and math teacher, and attendance at a Parent and Student Meeting. Students are chosen by a lottery. Once students are accepted into the academy, they must keep a good record of attendance, discipline, and academics. Students not meeting Academy requirements of a “C” or better in their elective class will be sent back to their zone school. Academy Elective courses include: Introduction to Health Careers, Health Science I & II, Forensics, Pharmacy Technician, Certified Nursing Assistant, Sports Medicine I, II, & III, First Responder, Emergency Medical Technician and Internship. During the student's 10th and 11th grade summers, they may be enrolled in AHEC of a summer program, which places students into various hospitals and allows them to experience health care related topics.
