• Every LPSS employee has an initial "start up" account for being an employee of the District.
• Choose investment options that meet your needs.
• Set an automatic deduction that drafts automatically from Payroll.
• Double check your Payroll to ensure its accuracy.

Change of Address Form
Use this form to change your name, address or telephone number. Once completed, please submit it to the Employee Services Department. 

LPSS Group Health Enrollment Form
Use this form to enroll in health insurance.

LPSS Group Health Insurance Change Form
Use this form to change your health insurance plan. Plan changes can only be made during Open Enrollment or due to a Special Enrollment event. View more information about Special Enrollment events. 

LPSS Group Health Cancellation Form
Use this form to cancel health benefits with LPSS.

Waiver of LPSS Group Health Insurance Form 
Use this form to voluntarily elect to decline the election of health insurance with LPSS. This form is MANDATORY if you are not choosing to elect health coverage with LPSS.

Health Savings Account (HSA) Change Form
Use this form to request a change in the amount you deduct from your check to be deposited into your HSA account.  You can change this deduction any time during the year.

The Group Health Plan of the Lafayette Parish School System is a self-funded plan whose claims are administered by a Third Party Administrator, BlueCross BlueShield of Louisiana. The Lafayette Parish School System offers three health plans to choose from. Employees may enroll in health plan benefits when hired, at open enrollment or during a "Special Enrollment Event" (as per the LPSS BlueCross BlueShield plan documents). Health Insurance benefits are effective on the date of hire based on election. ***Employees pay their Health Insurance premium a month in advance.

LPSS Humana Medicare Advantage Option

Retirees who are enrolled in Medicare Parts A and B also have an opportunity to elect the LPSS Humana Medicare Advantage Plan instead of the LPSS Blue Cross Blue Shield Health Insurance Plan. The primary advantages of joining the LPSS Humana Plan include lower premiums and additional benefits.

Retirees can add the LPSS Humana Medicare Advantage Plan at any time mid-year. This option allows you to try a different health insurance plan while still being able to return to the BCBS Group Health Insurance Plan. Should the LPSS Humana Medicare Advantage Plan not meet your needs, members may contact the LPSS Insurance & Benefits Department during the months of September – November and request a transition back to the LPSS BCBS Group Health Insurance Plan on January 1st.

IRMAA Penalty Reimbursements

High income earners, as determined by the Federal government annually, are charged a Medicare Part D surcharge under SilverScript. LPSS will reimburse members for any charge under Medicare Part D prescription coverage with proper documentation. Members are instructed to contact Kisha Gray / 337-521-7072 / kngray@lpssonline.com to work through the details of a Medicare Part D surcharge reimbursement.

Voluntary benefits are managed by Educational Benefits, Inc. (EBi) and are effective on the first of the month following (30) days of employment based on election. Benefits deduct monthly from the employee’s payroll.
