Transition Program

The Lafayette Parish School System Special Education Transition Department seeks to make students and their families aware of various community resources and to provide support to teachers as they work with students on the development of a plan to help prepare the student for their post-high school goals. The Lafayette Parish School System has an agreement with two main agencies to help identify students who may qualify for services and/or supports through the agency: Acadiana Area Human Services District and Louisiana Rehabilitation Services.

HOW LPSS TRANSITION STAFF WORKS WITH ACADIANA AREA HUMAN SERVICES DISTRICT:

  • Identify students, elementary through high school, with various developmental delays that manifest prior to age 22
  • Provide recommendations and agency information to families/students regarding contacting the agency to find out if the student currently qualifies for services/supports through the agency or if the student may qualify in the future.
  • Coordinate a meeting with a representative of the agency when possible.

HOW LPSS TRANSITION STAFF WORKS WITH LOUISIANA REHABILITATION SERVICES:

  • Coordinate Pre-Employment Training Service linkage meetings with a representative of the agency for interested high school junior and senior special education students who are capable of independent employment in the future. This program provides lessons in various soft skills related to employment including but not limited to writing resumes, interview skills and job searches.
  • Provide special education juniors the opportunity to participate in a work-based learning experience on a school campus during the summer following the student’s junior year of high school.
  • Provide special education seniors the opportunity to participate in a work-based learning experience on a school campus during the student’s senior year of high school.
  • Coordinate a linkage meeting with a representative of the agency beginning the junior year of high school for special education students identified as having significant hearing and/or visual impairments for the purpose of determining if the student will qualify for services that may be of benefit to the student as they transition from high school into either post-secondary school or employment.
  • Coordinate a linkage meeting with a representative of the agency during the senior/exit year for special education students who may qualify for services depending on the current categories being served by the agency for the purpose of determining if the student will qualify for services that may be of benefit to the student as they transition from high school into either post-secondary school or employment.
