Diet Prescriptions

Meal service to students with special dietary needs is provided to all school sites.  Federal and State Regulations require a completed, current Diet Prescription Form for any type of change/substitution to the posted menus.  The current school year Diet Prescription From for Meals at School is available below. Students with special dietary needs must have a current 2023-2024 diet prescription form filled out and submitted at the beginning of the school year.
